Registering for Lewis Middle School
We are happy to welcome you to our Lewis Middle School family! You are entering the nest of the 600+ strong Falcons. We will be glad to answer any questions if you e-mail them to the guidance department.
What to bring
Here are some suggested items to bring with you when you come to the
school to register. This will make the registration process go a lot
smoother.

Student's birth certificate

Student's Social Security card

Name and address of last school attended

Original Florida Certificate of Immunizations (form DH 680) prior to entering school

Florida physical; there is a 30-day grace period that begins the first day the student enters school. An existing physical will be accepted if conducted within one year prior to entering Lewis Middle School.

Exceptional Student Education records (if applicable)

Copy or proof of custody/guardianship (if applicable); Temporary Power of Attorney on Okaloosa County Special Power of Attorney and Certification form (if applicable)

Doctor's note for health concerns - Section 504 (if applicable)
