Girls Basketball Tryouts:
All grades - October 17, 18, and

Tryouts consist of testing players speed,
ball-handling and shooting ability. We conduct a summer basketball program
(June - July) and skills development camp (August - October).
Athlete/Parent Information
To be eligible for
basketball, you must have the following paperwork turned into the front office
by October 17, 2006. You can get parent permission forms and physical forms from
the school office. Please return them to the school office when completed.
Do not return them to Coach Payne.

Current sports physical which is good for
one year.

signature/date) PLEASE retain a copy for your records.

Parent permission form: MIS 4178.
This form must be notarized.
(Front office secretary is a notary)

First time athletes at Lewis must have a
copy of their birth certificate on file.
(parent can have the original copied in the front office)

Release for Use of Student Images in all Formats:
MIS 1171.

Maintain a 2.0 GPA or higher
throughout the winter sports season. No D's or F's
allowed.
Player Responsibilities
● Have all forms signed by parent and returned
to coach.
●
Be 110% committed to the Lady Falcon Basketball
program. Put team before self.
●
Give 110% effort at practice and games.
●
Attend tryouts and make the team.
●
Participate in all team functions.
●
You are expected to have
practice clothes each day. Shorts must be at least halfway to the top
of the knee and tank tops must have a tee-shirt underneath.
●
Notify the coach of your need to come late or miss a practice BEFORE it
occurs.
Tell the coach directly (don't send a message with a friend). Don't send
it with a teammate.
Leave a message on the coaches voice mail 833-4130 before noon.
Have the note placed in the coaches box in the front office before noon.

Player Eligibility
Requirements
● Age Requirement: A
perspective student athlete may not reach age 15 on or before August 31st.
● Academic Requirement
Students have six
semesters of eligibility
once they enter the sixth grade.
The perspective athlete should not have a grade lower than a "D" on the report
card or mid-nine weeks progress report.
The perspective athlete must have a 2.0 GPA or greater for her final yearly
average to qualify for participation for the
first nine weeks of school. The remainder of the year the
perspective athlete must maintain a 2.0 GPA or higher
each nine weeks.
To calculate the GPA assign the following numerical values to each of the
letter grades in each subject:
A= 4
B= 3 C= 2 D= 1 F=
0
Determine the sum of the grades (you should have six total) and divide by six.
