Athlete/Parent Information

To be eligible for basketball, you must have the following paperwork turned into the front office by October 17, 2005
 

  • Current sports physical which is good for one year.

  • Parent permission form ( This form must be notarized.)

  • First time athletes at Lewis must have a copy of their birth certificate on file.

  • Maintain a 2.0 GPA or higher throughout the winter sports season.

Lewis Middle School Basketball try-outs will be evaluated and chosen on the basis of a number of basketball related criteria.  An assessment of basic skills such as:

  • Shooting ability

  • Defense

  • Ability to make free throws

  • Ability to consistently make lay-ups

  • Ability to follow directions given by the coach

  • Ability to dribble with both hands proficiently

  • Dribble without looking at the ball.

  • Experience

  • Basketball instincts

The attitude of each player will be examined along with such things as:

  • Leadership

  • Coach ability

  • Dependability

  • Ability to be part of a team

  • Desire (Does the player act as if they want to be involved?)

  • Hustle
