School Advisory Council  Parent Ballot

For Parents of current 5^th, 6^th and 7^th Grade Students

Parents will be notified on the weekend telephone message system.

The School Advisory Council (SAC) By-Laws require Lewis Middle School parents to elect parent members who will serve on the SAC as representatives of the parent community. With that in mind, below you will find the following parents who have turned in their names for SAC membership consideration.

Parents - Please vote for two (2) candidates for the School Advisory Council (SAC).  The candidates receiving the most votes will become members of the SAC.  The remaining candidates will become alternate members.

You may print out a copy of the ballot and personally bring it to the Lewis Middle School office or you may email your ballot to reidm@mail.okaloosa.k12.fl.us
